Quality Control Policy Statement

The Quality Assurance Program maintained by BIO-GENEX LABORATORIES, INC. has the absolute and unqualified support of Laboratory Management. BIO-GENEX LABORATORIES INC. is firmly committed to professional personalized service and a program of constant quality improvement designed to provide our clients with the highest quality and most accurate data, at the lowest possible cost.

BIO-GENEX LABORATORIES INC personnel who will generate mineral/ore and water/wastewater quality data reporting and interpretations have big responsibility to those who will use it. Such data will become the basis for various action programs in water pollution control and exploration activity.

The goal of BIO-GENEX LABORATORIES INC is to maintain all necessary analytical process control to report FINAL RESULT as accurately and as confidentially as our analytical procedures and equipment technology will allow. Procedures and equipment shall be routinely evaluated and changes made, when necessary.

Sample Policy Acceptance

BIO-GENEX LABORATORIES INC. states its policy in sample acceptance. Each sample must be properly collected and submitted in a proper sample container or in standard condition. herbal fruit or plant products, mineral samples and/or water samples.

Policies on mineral samples must be dry, as well as the rocks, sand, mineral powder or a mixture of these. Wet or damp samples should be air-dried first before submitting to us. If the soil clumps forms clumps or sticks to the side of the container it is probably too wet for testing. Submitted ore samples must be 1 kilogram (1000 grams) in weight.

Samples for water/wastewater analyses are suggested to be submitted in a sterilized bottle containing a de-chlorinating chemical and must be kept cold (on ice) but not frozen. Make sure the ices’ don’t leak in shipment. Wastewater samples must be received at the Laboratory within two (2) hours after collecting the samples. If any above criteria are not met, it will postpone BIO-GENEX LABORATORIES INC. processing of samples, which could compromise “short” holding times and delay analysis. BIO-GENEX LABORATORIES INC. will contact the client regarding samples not meeting the sample acceptance policy. If the client cannot be immediately contacted, and hold times are not an issue, then samples will appropriately stored and will not be logged in until the situation is clarified with the client. However, if delay in sample log-in could result in missed hold times for any of the requested analyses, and if BIO-GENEX LABORATORIES INC. determines there is enough sufficient information provided on the Information Card, then BIO-GENEX LABORATORIES INC will attempt to log-in the samples to meet the hold time; however, BIO-GENEX LABORATORIES INC. will not be liable for any mistakes.

Required Documentation for Sample Information Card

Ore and Other Mineral samples must be in the appropriate sample bag and in exact amount of sample for the requested analysis. Samples are given a lot number which should never be repeated, name of mine; company or individual from whom received; the gross and net weights; as well as notes on the general character of the mineral.

Water and Wastewater samples must be in the appropriate container with adequate volume for the requested analysis, Waterproof or indelible ink must be used on labels / containers, Client Name, Address, Phone and Fax number, Contact Name, Sample Type, person submitting samples.

For regulatory reasons, BIO-GENEX LABORATORIES INC. will not accept samples with an incomplete sample information card. All fields of the sample information card must be completed before samples are accepted in the laboratory. In cases where fields of the sample information card lacks the required information or when samples show signs of damage or contamination. BIO-GENEX LABORATORIES INC. will contact the client, and the samples will be logged in for analysis after the situation has been resolved.

Sample Storages Policy

After initial testing, remaining samples of ore and wastewater that has been submitted by the clients are then properly labeled and stored for future references in case further analysis are being requested by the clients. Ore samples are stored within the duration of one year while wastewater sample stays only for 3 days in a refrigerator under certain temperature suitable for the wastewater samples.
